A comprehensive assessment carried out at the beginning of a client’s injury, shortly after the injury occurred, with a purpose of providing an assessment of physical, cognitive, and psychological symptoms, injuries and post-injury function in order to identify immediate care needs and interventions and to provide a basis for a comprehensive rehabilitation plan.
Case management is a collaborative, client-driven process for the provision of quality health and support services through the effective and efficient use of resources. Case management supports the client’s achievement of safe, realistic and reasonable goals within a complex health, social and fiscal environment.
A comprehensive report which sets out items, goods and services which have been identified as being reasonably necessary to attempt to restore the injured client back to the position he or she would have been in had the injury never occurred. The objective of such an analysis is to identify those items and services which may be required to maintain his or her physical, cognitive and psychological health, maximize his or her independence and safety, and replicate as closely as possible the lifestyle he or she would have experienced had the injury or illness not occurred. An analysis of the individual’s status and function prior to and subsequent to the injury or illness sets the stage for a determination of anticipated and related costs. A Cost of Future Care report lays the foundation for disability management throughout a person’s lifetime.
