- Coordinate and manage all rehabilitation services
- Liaise with team members, organize meetings of care team, monitor appointments and assist with planning and scheduling
- Hospital discharge planning and follow-up
- Recommend and put into place support services and equipment as required
- Accompany to medical appointments as appropriate
- Adapt strategies learned in a clinical setting to the home environment
- Regular reassessment of ongoing needs and new issues
